App is not opening anymore

This app is not working anymore. It has not opened on my 4s for over a week now. PLEASE update & post a new version ASAP! Thanks!!

Also, would be VERY helpful if you could add a TEMPLATE function to this app so photo info can be saved & not have to be typed every time. Thanks!!
